This role supports adult and family primary care visits, preventive care, chronic condition follow-up, patient education, and care coordination in the Palm Coast location.

• Evaluate patients, document visits, and support evidence-informed care plans.
• Partner with physicians, clinical staff, and office leadership to keep care timely and organized.
• Review medications, labs, referrals, and follow-up needs with a clear patient education approach.
• Maintain professional communication and strong clinical documentation standards.

• Room patients, collect vitals, update medication lists, and prepare patients for visits.
